Article Abstract
Progressive encephalomyelitis with rigidity and myoclonus (PERM) is a rare and severe syndrome characterized by rigidity of the limb and truncal muscles, brainstem signs, myoclonus, and hyperekplexia. Iliopsoas hematoma is a serious complication of bleeding disorders that occurs most commonly in patients with hemophilia and also in association with anti-coagulant drug treatment. We herein present a case of PERM complicated with bilateral iliopsoas hematomas. His neurological symptoms improved after immunotherapy, and thereafter the iliopsoas hematomas disappeared. Neurologists should consider iliopsoas hematomas as a serious potential complication of PERM.

This case concerns a 66-year old man with lower abdominal and groin pain and bilateral spontaneous flank ecchymoses, based on a spontaneous bilateral iliopsoas hematoma. This case illustrates that, although uncommon, an iliopsoas hematoma can be the cause of a Grey Turner sign, indicating a retroperitoneal haemorrhage.

We present a case of invasive pneumococcal disease (IPD) complicated by both iliopsoas abscess and pyogenic vertebral osteomyelitis caused by serotype 35F in a 64-year-old man with a history of splenectomy who was unvaccinated. The patient experienced difficulty moving, severe back pain, vomiting, and high fever. Advanced imaging techniques, including T2-weighted lumbar MRI and diffusion-weighted whole-body imaging with background body signal suppression (DWIBS), revealed hyperintensities in the left iliopsoas muscle and L4-L5 vertebral bodies, facilitating diagnosis.

Regular physical activity, including resistance training, is strongly recommended for older adults to preserve muscle mass, balance, and functional independence. However, the increasing use of direct oral anticoagulants (DOACs), such as apixaban, in this population introduces important clinical considerations, particularly the risk of spontaneous bleeding in those participating in physical activity. We report a case of idiopathic retroperitoneal hematoma in an 82-year-old man with moderate COVID-19 pneumonia receiving prophylactic anticoagulation therapy. The patient was initially treated with remdesivir, dexamethasone, and subcutaneous heparin, showing improved respiratory function. On hospital day 6, he developed sudden right lower abdominal pain without prior trauma.
