Cutaneous reactive angiomatoses encompass a spectrum of conditions driven by underlying occlusive vasculopathy. We present 2 cases of reactive angioproliferation in the context of end-stage renal failure (ESRF) manifesting as painful cutaneous ulceration. The first case demonstrates histologic features of diffuse dermal angiomatosis. The second case illustrates a nonspecific pattern of vascular proliferation which does not conform to any of the classically described subtypes, therefore reinforcing the concept of a spectrum of histopathologic changes in reactive angioproliferations. Diffuse dermal angiomatosis has been described in ESRF alone and in association with calciphylaxis, a life-threatening condition. Recognizing the patterns of reactive angioproliferation in the context of ESRF therefore has clinical relevance. Earlier identification may facilitate mitigation of risk factors and improve prognosis.

Introduction: Cutaneous manifestations in adult-onset immunodeficiency (AOID) resulting from anti-interferon-gamma autoantibody (AIGA) are prevalent and can be classified into infective and reactive disorders. Intravascular hemolysis (IVH), a pathological process associated with various conditions, triggers inflammatory responses, yet the key molecular drivers of these responses are poorly defined, particularly within the vasculature. To explore the role of NLRP3 inflammasome- and caspase-1-dependent pathways in IVH-induced vascular dysfunction, we used models of acute and chronic IVH, alongside heme stimulation of endothelial cells, thereby isolating this disease mechanism from its etiological causes. IVH induced rapid inflammatory responses in C57BL/6J mice, including IL-1β release within 15 minutes, and NLRP3-dependent caspase-1 activation in circulating leukocytes.

Post-occlusive reactive hyperemia (PORH) is a physiological response marked by a transient increase in microvascular perfusion following ischemia. While cutaneous perfusion during PORH has been extensively characterized using optical approaches such as Doppler-based techniques, low-cost alternatives like photoplethysmography (PPG), videocapillaroscopy (VC) and near-infrared reflectance imaging (NIRI) may provide complementary insights into both microvascular and venous dynamics. However, their role in quantifying PORH remains underexplored.

Nonsteroidal anti-inflammatory drug (NSAID) hypersensitivity reactions are commonly reported but often overestimated due to reliance on clinical history alone. Accurate diagnosis and identification of safe alternative medications are essential for appropriate management. This retrospective study aimed to evaluate the clinical manifestations of NSAID hypersensitivity, assess the diagnostic value and safety of aspirin oral provocation testing, and investigate the tolerability of alternative medications, including acetaminophen, meloxicam, and celecoxib.
