Background: With increasing frequency, providers are encountering patients with opioid overdose who recrudesce after intermittent bolus dosing of naloxone. Some patients require a continuous infusion to maintain ventilation, which necessitates admission to a monitored setting. Buprenorphine could shorten the duration of a continuous naloxone infusion (CNI) or preclude the need altogether because its long-lasting partial agonist effects compete with and blunt the respiratory depressant effects of full agonist opioids.

Background: Most opioid use disorder (OUD) treatment guidelines target community medical settings, and the subsequent recommendations were established to prioritize safety and reduce diversion prior to the fentanyl era. For people with OUD who use unregulated fentanyl, slow induction onto opioid agonist therapy (OAT) with gradual dose titration is often ineffective or insufficient for reducing withdrawal symptoms and cravings, thereby hampering engagement and retention in treatment. Given the severe risks associated with continued use of the increasingly toxic unregulated drug supply, new and innovative approaches to the management of OUD are urgently needed.

Alfaxalone has been studied for anesthetic induction of rabbits with rapid onset and a short duration of action; however, it has been minimally evaluated as an option for anesthetic maintenance. This study compared alfaxalone-based total intravenous anesthesia maintenance protocols against inhaled isoflurane, the current standard for anesthetic maintenance in rabbits.
