Tumor-induced osteomalacia (TIO) is a rare paraneoplastic syndrome of abnormal phosphorus metabolism caused by increased secretion of fibroblast growth factor 23 (FGF23) by small mesenchymal tumors. In this article, we reported a patient with chronic refractory hypophosphatemia due to TIO who is treated with burosumab, a monoclonal antibody that targets and blocks the activity of FGF23. Treatment with burosumab led to the resolution of his refractory hypophosphatemia, but this was complicated by the development of secondary hyperparathyroidism.

Introduction: Near-total parathyroidectomy (NPTX) could provide persistent improvements in dysregulated mineral metabolism, leading to a reduction in all-cause and cardiovascular (CV) mortality by reducing coronary artery calcium score (CACS) in refractory secondary hyperparathyroidism (SHPT). In this study, we have attempted to compare the effect of NPTX and cinacalcet therapy in patients with refractory SHPT on dialysis, with regard to their effects on CACS and FGF-23.
Methods: A total of 14 patients with refractory SHPT were followed prospectively.

Renal ischemic disease represents a severe clinical pathological condition commonly observed in acute kidney injury (AKI), renal transplantation, and kidney surgery. It leads to renal tubular epithelial cell damage, inflammatory responses, and cell death, potentially progressing to chronic kidney disease (CKD) or even renal failure, significantly impairing patients' quality of life and survival rates. Current therapeutic strategies for renal ischemia-reperfusion injury (IRI) include pharmacological interventions, cell therapy, and gene therapy, yet their efficacy remains limited and may be accompanied by adverse effects.

While extensive studies have elucidated their critical roles in morphological regulation, how these signals orchestrate the epigenome to confer developmental competence remains unclear. In this study, we demonstrate that H3K9me3-marked facultative heterochromatin domains undergo global reorganization during differentiation of human pluripotent stem cells into mesoderm and endoderm, which arise through epithelial-mesenchymal transition (EMT), but not into ectoderm, which retains epithelial state.
