Article Abstract

Introduction: There is widespread variation in the management of rare orthopaedic disease, in a large part owing to uncertainty. No individual surgeon or hospital is typically equipped to amass sufficient numbers of cases to draw robust conclusions from the information available to them. The programme of research will establish the British Orthopaedic Surgery Surveillance (BOSS) Study; a nationwide reporting structure for rare disease in orthopaedic surgery.

Methods: The BOSS Study is a series of nationwide observational cohort studies of pre-specified orthopaedic disease. All relevant hospitals treating the disease are invited to contribute anonymised case details. Data will be collected digitally through REDCap, with an additional bespoke software solution used to regularly confirm case ascertainment, prompt follow-up reminders and identify potential missing cases from external sources of information (i.e. national administrative data). With their consent, patients will be invited to enrich the data collected by supplementing anonymised case data with patient reported outcomes. The study will primarily seek to calculate the incidence of the rare diseases under investigation, with 95% confidence intervals. Descriptive statistics will be used to describe the case mix, treatment variations and outcomes. Inferential statistical analysis may be used to analyze associations between presentation factors and outcomes. Types of analyses will be contingent on the disease under investigation.

Discussion: This study builds upon other national rare disease supporting structures, particularly those in obstetrics and paediatric surgery. It is particularly focused on addressing the evidence base for quality and safety of surgery, and the design is influenced by the specifications of the IDEAL collaboration for the development of surgical research.

Importance: Children of marginalized racial groups have poorer surgical outcomes compared to White children. Clinical communication may contribute to these disparities.

Objective: We explore racial differences in parent-clinician communication during initial consultations for elective surgical procedures in children.

While both prosocial teases and aggressive jokes are characterized by a humorous provocation, a tease is intended to communicate face-protective information or general affiliation while an aggressive joke is intended to socially demean the target. Thus, when targeted by a joke, people must judge whether the intention behind the joke is affiliative or aggressive. To interpret the intention behind a joke, the target must consider information such as "off-record marker" usage and the reputation of the joker.

Renal dysfunction causes dyslipidemia, and the progression of kidney disease is associated with treatment-requiring lipid disorders. However, there is little data on the association between the severity of dyslipidemia and various treatment modalities with renal function. A total of n = 214 prevalent patients from a lipidology and nephrology practice were investigated in an unicentric cross-sectional study and divided into 4 groups based on the severity and therapeutic regimen of an existing lipid metabolism disorder: a lipid apheresis-treated group (LA), a drug-treated group (MG), a control group (CG) that included patients with lipid disorder not needing any treatment, and a diabetes group that comprised all diabetes patients.

A new baurioid therocephalian, Jiucaiyuangnathus confusus, was recently described from the Lower Triassic Jiucaiyuan Formation of Jimusar, Xinjiang, China. Here, two or three newly collected specimens from the same horizon of Turpan, Xinjiang, including a complete skull with lower jaw and partial skeletons, are referred to this species. The new skull is slightly larger than that of the holotype.

White-tailed deer () in the wild and on cervid farms have drawn the attention of state wildlife agencies and animal health agencies as Chronic Wasting Disease (CWD) has spread across North America. Deer farm regulations have been implemented to reduce direct contact between wild and farmed cervids; however, evidence suggests that indirect contact to infectious prions passed through the alimentary tracts of scavengers may be an important transmission pathway. The objective of this study was to characterize mammalian scavenger and wild deer activities associated with deer farms and link these activities with site-specific spatial covariates utilizing a network of camera traps, mounted to farm perimeter fences.
