Background: The ANTHEM-HF pilot study was an open-label study that evaluated the safety and feasibility of autonomic regulation therapy (ART) utilizing cervical vagus nerve stimulation (VNS) for patients with chronic HF with reduced EF (HFrEF). Patients in NYHA class II-III with EF ≤40% (n = 60) received ART for 6 months post-titration. ART was associated with sustained improvement in left ventricular (LV) function and HF symptoms at 6 and 12 months.
Methods: Continuously cyclic VNS was maintained to determine longer-term safety and chronic effects of ART. Echocardiographic parameters and HF symptoms were assessed throughout a follow-up period of at least 42 months.
Results: Between 12 and 42 months after initial titration, there were no device-related SAEs or malfunctions. There were 10 SAEs adjudicated to be unrelated to VNS, including 5 deaths. There were 6 non-serious adverse events that were adjudicated to be device-related (2 oropharyngeal pain, 1 implant site pain, 2 voice alteration, and 1 hoarseness). At 42 months, there was significant improvement from baseline in LVEF, NYHA class, 6-min walk distance, and MLHFQ score. However, these improvements at 42 months were not significantly different from mean values at 6 and 12 months.
Conclusions: In a 42-month follow-up, ART was durable, safe, and was associated with beneficial effects on LVEF and 6-min walk distance. Long term, chronic, open-loop ART continued to be well-tolerated in patients with HFrEF. The open label, randomized, controlled, ANTHEM-HFrEF Pivotal Study is currently underway to further evaluate ART in patients with advanced HF.

Sex hormones influence blood pressure regulation through interaction with several main regulatory systems, including the autonomic nervous system, the renin-angiotensin-aldosterone system, endothelin, and renal mechanisms. The modulation of vascular function by sex hormones varies over the lifespan.

Takotsubo Cardiomyopathy (TC) is a transient cardiomyopathy secondary to emotional and/or physical stress. While its precise aetiology remains unclear, some evidence suggests a possible role for the insular cortex (IC), which modulates cardiovascular responses to stress. The IC is a key viscerosensory and visceromotor hub with widespread connections, and is implicated in interoceptive processing, emotional regulation, and autonomic control.

Chronic diarrhea is a frequent gastrointestinal complication in both type 1 (T1D) and type 2 diabetes (T2D), although the underlying mechanisms differ: T1D is linked to autonomic neuropathy and disrupted transporter regulation, while T2D is often linked to medications and intestinal inflammation. Using streptozotocin-induced mouse models of T1D and T2D, we observed increased luminal fluid in the small intestine of both. Given the role of Na⁺/H⁺ exchanger 3 (NHE3) in fluid absorption and its loss in most diarrheal diseases, we examined NHE3 expression across intestinal segments.

Pelvic visceromotor functions such as micturition are regulated by coordinated autonomic and somatic motor pathways from the central nervous system. The parasympathetic system induces detrusor muscle contraction while the somatic system facilitates relaxation of the external urethral sphincter, ensuring synchronized and efficient bladder emptying during the voiding process. This study explores the relationship between Barrington's nucleus corticotropin-releasing hormone (CRH)-ergic projections and the formation of perineural nets (PNNs) among spinal motoneurons, particularly parasympathetic preganglionic neurons in the intermediolateral nucleus (IML) and Onuf's nucleus during the maturation of the neural circuitry controlling pelvic visceromotor functions.
