Article Abstract

Objective: Persistent post-surgical pain is common among patients undergoing surgery, is detrimental to patients' quality of life, and can precipitate long-term opioid use. The purpose of this randomized controlled trial is to assess the effects of a behavioral intervention offered prior to surgery for patients at risk for poor post-surgical outcomes, including persistent pain and impaired functioning.

Methods: Described herein is an ongoing randomized, patient- and assessor-blind, attention-controlled multisite clinical trial. Four hundred and thirty Veterans indicated for total knee arthroplasty (TKA) with distress and/or pain will be recruited for this study. Participants will be randomly assigned to a one-day (~5 h) Acceptance and Commitment Therapy workshop or one-day education and attention control workshop. Approximately two weeks following their TKA surgery, patients receive an individualized booster session via phone. Following their TKA, patients complete assessments at 1 week, 6 weeks, 3 months, and 6 months.

Results: The primary outcomes are pain intensity and knee-specific functioning; secondary outcomes are symptoms of distress and coping skills. Mediation analyses will examine whether changes in symptoms of distress and coping skills have an impact on pain and functioning at 6 months in Veterans receiving ACT. This study is conducted mostly with older Veterans; therefore, results may not generalize to women and younger adults who are underrepresented in this veteran population.

Conclusions: The results of this study will provide the first evidence from a large-scale, patient- and assessor-blind controlled trial on the effectiveness of a brief behavioral intervention for the prevention of persistent post-surgical pain and dysfunction.

Anomalous pulmonary venous connection (APVC), including total (TAPVC) and partial (PAPVC) forms, is a congenital heart defect with abnormal pulmonary vein drainage; and while surgical repair has improved survival, its long-term impact on cardiopulmonary function remains unclear. This retrospective study evaluated exercise capacity and pulmonary function in 26 pediatric APVC patients (17 TAPVC, 9 PAPVC) using cardiopulmonary exercise testing (CPET) and compared them with 63 age-matched healthy controls. Patients with complex defects or significant comorbidities were excluded.

Acceptability and feasibility of a cognitive-behavioural intervention for pain management before total knee arthroplasty: a pilot trial.

Psychol Health

September 2025

Departmentt of Anaesthesiology and Perioperative Medicine, Waitematā Pain Services, Te Whatu Ora, Waitematā, Auckland, New Zealand.

Background: Although psychological factors predict the development of persistent pain after total knee arthroplasty (TKA), psychological interventions to prevent persistent pain are under-explored. This pilot trial evaluated the acceptability and feasibility of a cognitive-behavioural intervention targeting risk factors for post-surgical pain among patients scheduled for TKA (UTN: U1111-1243-1067/ACTRN12621001095853).

Methods: Patients with elevated expectations of post-surgical pain and anxiety were recruited from TKA waiting lists.

Telerehabilitation After Surgery in Adolescent Idiopathic Scoliosis: A Randomized Controlled Trial.

Healthcare (Basel)

August 2025

Advanced Spine Surgery Center, Department of Orthopedic Surgery and Traumatology, Emsey Hospital, 34912 İstanbul, Turkey.

: Structured postoperative rehabilitation is not routinely provided for individuals with adolescent idiopathic scoliosis (AIS) after surgery, with physiotherapy typically limited to the immediate inpatient period. Telerehabilitation offers an accessible and supervised option to address persistent functional limitations, pain, and quality-of-life concerns in this population. : This study aimed to evaluate the effects of a synchronous telerehabilitation program-designed to support post-surgical recovery in individuals with adolescent idiopathic scoliosis (AIS)-on trunk muscle endurance, trunk flexibility, functional capacity, pain severity, perception of appearance, and quality of life.

The effects of long-term adjustments in body weight on the lipid balance in patients with severe obesity are not well understood. This study aimed to evaluate a non-invasive lipidomic approach to identifying biomarkers that could help predict which patients may require additional therapies before and after weight loss. Using mass spectrometry, 275 lipid species were analysed in non-obese controls, patients with severe obesity, and patients one year after bariatric surgery.

Background: Hidradenitis suppurativa (HS) is a chronic inflammatory skin condition often requiring surgical intervention for definitive treatment. Previous studies evaluated post-surgical outcomes, but no standardization exists for collection and nomenclature for HS surgical outcomes.

Objective: To characterize and define surgical outcome terminology.
