Background: Current recommendations for a prophylactic (primary prevention) implantable cardioverter defibrillator (ICD) in patients with both ischemic and non-ischemic heart failure with reduced ejection fraction (HFrEF) originate from clinical trials conducted in selected patients over 20 years ago that showed an overall statistically significant survival benefit associated with a primary prevention ICD in the range of 23%-34%. The recent introduction of angiotensin receptor-neprilysin inhibitors [ARNI] and sodium glucose co-transporter 2 inhibitors [SGLT2i]) was shown to further reduce the risk of sudden cardiac death (SCD) in patients with HFrEF. Thus, there is an unmet need appropriately designed comparative effectiveness clinical trials aimed to reassess the survival benefit of a primary prevention ICD in contemporary patients with HFrEF.

A central paradigm in cardiac biology is the reactivation of the fetal gene programme in the adult heart in response to stress. This so-called 'fetal gene hypothesis' was first proposed almost 40 years ago following the observation that certain fetal contractile protein isoforms were re-expressed in hypertrophied ventricles in the rodent heart in response to haemodynamic overload. Consequently, this concept was broadly adopted, and activation of the fetal gene programme became synonymous in the literature with the cardiac stress response.

Before the 2000s, the antifungal toolbox against invasive fungal infections (IFIs) was only composed of very toxic amphotericin B, weakly selective and nearly unused 5-fluorocytosine and azoles that have been massively used in agriculture and horticulture. The emergence of resistances followed their respective launch, so many antifungals became progressively inefficient whereas the antifungal research was left behind. In the 2000s, echinocandins (ECs) were introduced as a new antifungal class of natural origin and of unprecedented mechanism of action.
