Article Abstract

Background And Aim Of The Study: Although abundant biological, clinical, and scientific evidence exists on the superiority of multi-arterial (MAR) and total-arterial revascularization (TAR) over the conventional strategy with a single internal thoracic artery, only 10% of patients undergoing coronary artery bypass grafting (CABG) in the United States receives a second arterial conduit, and only 5% of patients receives TAR.

Methods And Results: In January 2020, the authors performed comprehensive search to identify studies that evaluated MAR and TAR strategies through the MEDLINE database.

Conclusions: In this paper, the authors reviewed the literature on the historical and current evidence in favor of MAR and TAR, thus underlying why current CABG practice needs qualitative improvement.

Nearshore marine sediments in a Puget Sound, Washington industrial embayment had elevated levels of PAHs, PCBs and DDTs. Chemical fingerprints implicated nearshore sources including creosote, industrial oil and tar waste, and a landfill. Elevated concentrations were confined to an approximate 300-m shoreline buffer in the industrial waterfront, suggesting high site fidelity and limited along-shore or off-shore transport.

Plastic litter is ubiquitously distributed across the seafloor, yet the transport trajectories and settling mechanisms of such debris remain poorly defined. In this study, we conducted a comprehensive analysis of seabed litter collected via bottom trawls at depths ranging from 80 to 1300 m in the southeastern Mediterranean Sea, an oligotrophic marine system. Our results revealed one of the highest global concentrations of deep-sea plastic litter (up to 3321 ± 12 items/km), with plastic bags and packaging (PBPs) comprising over 91 % of total items.

Objective: We sought to review the outcomes of our arch-first total aortic arch repair (TAR) using a trifurcated graft after previous acute type A aortic dissection (ATAD) repair.

Methods: From February 2006 to June 2024, 62 patients underwent reoperative TAR after ATAD repair. The first-stage TAR includes axillary artery cannulation, minimal dissection without aortic crossclamping, myocardial protection using systemic potassium and retrograde blood cardioplegia, an arch-first technique with deep hypothermia (20 °C), and construction of a classical elephant trunk through a partial transverse incision distally or proximally to old distal aortic anastomosis.
