Pediatric choledocholithiasis is a rare but increasingly recognized disease process that is predominantly managed with either an "endoscopy-first" or "surgery-first" (SF) approach. In the endoscopy-first approach, a preoperative endoscopic retrograde cholangiopancreatography is performed with subsequent cholecystectomy. In contrast, the surgery-first approach is a single-stage procedure that includes a laparoscopic cholecystectomy and intraoperative cholangiogram, followed by a laparoscopic common bile duct exploration when indicated.

Addressing antimicrobial resistance requires multi-disciplinary action, including from the clinical laboratory. Cascade reporting of the antimicrobial susceptibility test (AST) is a strategy used by some laboratories to nudge clinicians toward the use of more narrow-spectrum antimicrobials. Cascade reporting involves suppression of broader-spectrum antimicrobials if the narrower-spectrum first-line antimicrobials show in-vitro susceptibility.

Bacteria defend themselves from viral predation using diverse immune systems, many of which target foreign DNA for degradation. Defence-associated reverse transcriptase (DRT) systems provide an intriguing counterpoint to this strategy by using DNA synthesis instead. We and others recently showed that DRT2 systems use an RNA template to assemble a de novo gene that encodes the antiviral effector protein Neo.

The 2024 American College of Surgeons Point Counterpoint conference held in Baltimore, Maryland included a panel on pediatric trauma. The panelists included pediatric surgeons, a pediatric trauma nurse practitioner, a trauma pharmacist and a trauma surgeon. The discussion focused on unique clinical considerations for pediatric abdominal trauma, pharmacologic best practices and optimizing care models to incorporate pediatric and trauma surgeons.
