Systemic mastocytosis (SM) is a rare clinical condition resulting from a clonal proliferation of abnormal mast cells. The clinical presentation may vary from mild cutaneous manifestations to aggressive systemic symptoms including intermittent episodes of anaphylaxis. We present a case of a 69-year-old male with abrupt and recurrent episodes of anaphylaxis with refractory distributive shock following cardiac surgery with cardiopulmonary bypass. Following a complex postoperative course, a bone marrow biopsy ultimately confirmed the diagnosis. Although rare, SM should be considered in the differential diagnosis of postoperative patients with unexplained and recurrent episodes of distributive shock.

malaria remains a major cause of morbidity and mortality, particularly in endemic regions. We report the case of a 21-year-old male with recent travel to an endemic area (Guapi, Colombia), who presented with febrile symptoms, severe respiratory distress, and oxygen saturation below 75%, necessitating orotracheal intubation. During the procedure, he developed pulseless electrical activity cardiac arrest, achieving return of spontaneous circulation after advanced resuscitation.

Methylene blue (MB) is a versatile medicine with a long history of use in various medical applications, including dye, antiseptic, and treatment for methemoglobinemia. It has a role in vasoconstriction, methemoglobin reduction, inhibition of guanylate cyclase, and anti-inflammatory effects. Purpose Of Review: The rising incidence of anaphylaxis and reports of cases unresponsive to standard epinephrine treatment highlight the relevance of refractory anaphylaxis. There is a lack of standardized management protocols, and clinicians across specialties need guidance to handle this rare but life-threatening condition.
Recent Findings: There is a lack of a universally accepted definition, with most studies describing it as anaphylaxis unresponsive to several doses of intramuscular epinephrine.

Objectives: Guiding fluid therapy in severely burned patients presents complex challenges, combining elements of both distributive and hypovolemic shock, accompanied by cardiovascular dysfunction. Whereas parts of hemodynamic assessment in septic shock such as central venous oxygen saturation (ScvO2) and the arterial-venous carbon dioxide (CO2) partial pressure difference (ΔpCO2 = pcvCO2 - paCO2) can both be used as indicators of hypoperfusion, they are not part of the standard clinical assessment of severely burned patients. The aim of this retrospective study was thus to investigate whether the ΔpCO2 and ScvO2 in severely burned patients correlates with fluid administration and mortality.

Case Presentation: We report the case of a 65-year-old Caucasian woman who presented to our emergency department following the ingestion of approximately thirty 2.5 mg bisoprolol tablets. Initially stable, she rapidly developed severe bradycardia and hypotension, culminating in cardiogenic-distributive shock that required intubation, aggressive vasoactive support, and advanced metabolic therapies.
