Positional nystagmus is a frequently encountered yet often underappreciated clinical sign that provides critical insights into vestibular and central nervous system function. For the general neurologist, recognising and correctly interpreting positional nystagmus can significantly impact diagnostic accuracy and guide appropriate management of common and complex dizziness presentations. The current diagnostic framework for positional nystagmus disproportionately favours BPPV, underestimates central positional nystagmus (CPN), and over-relies on imaging.

Unlabelled: Neonatal status epilepticus (NSE) is associated with poor survival and adverse neurological outcomes. However, current definitions only partially account for the unique pathophysiology of the neonatal brain and the clinical context of acute symptomatic seizures. To address this gap, international efforts are underway to develop a more specific and context-appropriate definition for the neonatal period.

Dr John Laragh, a pioneer in the field of hypertension, held a fundamental belief in the need to challenge existing dogmas in medicine to enhance our scientific understanding of disease and advance patient care. Perhaps in no area of hypertension does this ring truer with than primary aldosteronism (PA). Following its initial description in the mid-1950s with an initial surge in diagnoses soon thereafter, PA was ultimately relegated to be considered a "zebra" of hypertension felt to be responsible for only a small percentage of cases.

This commentary re-examines recent proposals to define quality in qualitative research through a singular unifying framework, situating them alongside historical and ongoing debates in qualitative methodology. By juxtaposing different traditions, this piece highlights areas of tension between procedural notions of rigour and interpretive approaches that emphasise the co-constructed, context-bound nature of meaning. The discussion argues that quality in qualitative research cannot be captured by a single metric or universal rule.
