Reconstructing multi-tissue defects in the finger remains a significant challenge in hand surgery. We present the case of a 37-year-old man with segmental loss of bone, skin and extensor apparatus on the dorsal aspect of the index finger. A single stage reconstruction was successfully performed using a pedicled chimeric flap based on the second dorsal metacarpal artery combining skin paddle, second metacarpal base bone and the extensor indicis proprius.

Background: Extensor indicis proprius (EIP) transfer augmented with proximal extensor pollicis longus (EPL) stump lengthening restores thumb extension and optimizes function in cases of chronic EPL tendon ruptures, which impair hand dexterity and fine motor skills. Traditional EIP-to-EPL transfers often disrupt the natural oblique course of the EPL around the Lister tubercle, leading to functional deficits. This dual-tendon transfer preserves anatomical alignment and improves thumb biomechanics, enhancing extension strength and the adduction moment arm at the carpometacarpal (CMC) joint.

Case: A 36-year-old soldier presented for foreign training fitness with a history of a fall 18 years previously, that injured his nondominant left wrist. Initially, the injury was managed conservatively with a below elbow cast. Since then, he had been unable to extend the thumb at the interphalangeal joint but adapted well to daily activities.

Background: Tendon reconstruction is a difficult procedure with unpredictable results, and evaluating and selecting the appropriate muscle-tendon is crucial. To improve the results of tendon reconstruction, we investigated active muscle contraction during wide-awake local anesthesia no tourniquet (WALANT) surgery, in which the patient can perform active movements.
Methods: Among 213 WALANT tendon reconstructions, 148 muscle-tendons were evaluated for active contraction distance and passive stretch distance of the muscle-tendons, time elapsed before reconstruction, and characteristics and changes in muscle active contraction patterns after verbal instructions.

Beckwith-Wiedemann syndrome is a rare congenital genetic condition with various clinical manifestations, notably isolated lateralized overgrowth. Literature regarding hemihyperplasia of the hand and upper extremity is scarce. We present a 9-year-old right handedgirl with right upper-extremity hemihypertrophy who was followed extensively over 9 years for various soft tissue complaints.
