Article Abstract
We report a child with Lennox-Gastaut syndrome with an increase in seizure frequency and loss of psychomotor skills due to a disintegrated cervical VNS lead, not detected during standard device monitoring. The lead was completely removed and replaced by a new 303 lead on the same nerve segment. After reinitiating VNS, side effects forced us to switch it off, resulting in immediate seizure recurrence. EEG recording demonstrated a non-convulsive status epilepticus that was halted by reinitiating VNS therapy. Thereafter, he remained seizure free for eight months, and regained psychomotor development.

Developmental and epileptic encephalopathies (DEEs), including Infantile Epileptic Spasms Syndrome (IESS) and Lennox-Gastaut Syndrome (LGS), are severe pediatric conditions characterized by profound developmental delays and treatment-resistant epilepsy. Although steroid therapies provide some clinical benefits, the underlying immunological mechanisms remain poorly understood. Deep brain stimulation (DBS) targeting the centromedian (CM) thalamic nucleus is an emerging treatment option for patients with Lennox-Gastaut syndrome. Ten LGS patients with CM-DBS underwent one-hour scalp EEG approximately one-month post-surgery. Following a 10-minute baseline EEG, stimulation commenced at 1V for 5 minutes, increasing in 1V increments up to 5V or until paresthesias occurred.

Plant-derived, highly purified CBD oral solution (100 mg/mL) is approved as Epidiolex in the United States and as Epidyolex in Europe for the treatment of seizures associated with Lennox-Gastaut syndrome, Dravet syndrome, or tuberous sclerosis complex with country-specific labels.
