Article Abstract

Rhabdopeptides are a large class of nonribosomal peptides from the bacteria Xenorhabdus and Photorhabdus with low micromolar activity against different protozoa, which are the causative agents of several tropical diseases. The development of a facile and flexible synthesis combining backbone amide linking with on-resin peralkylation for the synthesis of permethylated rhabdopeptides is described. This strategy allows the fast generation of permethylated naturally occurring and artificial rhabdopeptides for a structure-activity study. Furthermore, in vitro experiments revealed their superior properties regarding their stability and passive membrane diffusion.

The pH Low Insertion Peptide (pHLIP), a cell-penetrating peptide, presents an ideal model to study peptide-membrane interactions across a range of conformational states. The folded, solvent-exposed unfolded, and membrane-inserted states of pHLIP have been well-characterized, but the intermediate structures remain poorly understood. Studies have focused on understanding folding and membrane interactions; however, there is a relation between the environment, membrane interactions, and local picosecond dynamics that has not been characterized.

Background: Low back pain is a prevalent issue with various etiologies, one of which includes pars interarticularis (pars) defects. While traditional fluoroscopic guidance has been the standard for administering injections, both fluoroscopic and ultrasound-guided injections for the management of low back pain have been shown to be similar in efficacy. The benefits of ultrasound guidance include real-time visualization, reduced radiation exposure, and improved patient access.

General anesthesia (GA) is often preferred for cervical spine surgeries; however, it may pose risks for patients with severe comorbidities. This case report describes management of cervical decompression laminectomy using continuous thoracic spinal anesthesia (TSA) with hypobaric levobupivacaine in a high-risk elderly patient. This technique provided an effective alternative to GA.
