Article Abstract

The importance of fibrinogen replacement therapy in obstetric disseminated intravascular coagulation is well recognized. However, fibrinogen measurement in conventional laboratories has been a time-consuming task. Recently, a Japanese manufacturer developed a portable device that enables immediate fibrinogen measurement at the point of care. This report describes a case in which this device was used for the successful management of obstetric disseminated intravascular coagulation.

Although it is difficult to achieve expertise in surgical anatomy of the female perineum, this is essential for patient counseling, clinical decision making, and surgical safety. Education of female pelvic anatomy has often relied on historical illustrations that contain inaccuracies and omissions. Therefore, the Society of Gynecologic Surgeons Pelvic Anatomy Group was established to promote accuracy through standardization of terminology, to explore anatomy through three-dimensional magnetic resonance imaging (MRI) technology, and to disseminate knowledge through the development of a website that uses the human body as a primary source of education.

Disseminated intravascular coagulation (DIC) associated with critical obstetrical hemorrhage and severe trauma is classified as fibrinolytic DIC in terms of pathology and acute DIC in terms of progression. Obstetrical DIC is triggered by the influx of tissue factors from the placenta, amniotic fluid, and decidua into the maternal circulation. In contrast, trauma-related DIC is caused by vascular endothelial damage and exposure of subendothelial tissue.

Introduction: Facing the profound health impacts and economic challenges posed by the high maternal mortality ratio worldwide, point-of-care ultrasound (POCUS) has shown its potential to enhance diagnostic efficiency, reduce healthcare costs and improve health outcomes. However, concerns regarding the explicit and implicit costs as well as actual clinical outcomes of POCUS also exist. This systematic review protocol aims to synthesise existing evidence regarding the costs and cost-effectiveness of POCUS in obstetric care to provide a clearer understanding of its economic and clinical impact, ultimately guaranteeing global maternal and neonatal health and supporting the vision of the 2030 sustainable development goals.
