Vertebrobasilar dolichoectasia (VBD) is characterized by significant dilation, elongation, and tortuosity of the vertebrobasilar system. We present a unique case of VBD, confirmed by neuroimaging studies, showing vascular compression of the right optic tract and lower cranial nerves leading to an incongruous left homonymous inferior quadrantanopia and glossopharyngeal neuralgia.

Objective: The purpose of this study was to evaluate the risk of selected adverse outcomes for older adults with a new episode of neck pain (NP) receiving chiropractic care compared to those receiving primary medical care with Prescription Drug Therapy (PDT) or primary care without medication.
Methods: Through analysis of Medicare claims data, we designed a retrospective cohort study including 291 604 patients with a new office visit for NP in 2019. We developed 3 mutually exclusive exposure groups: the Chiropractic Manipulative Therapy (CMT) group received spinal manipulative therapy from a chiropractor with no primary care visits; the PDT group visited primary care and filled an analgesic prescription within 7 days without chiropractic care, and the Primary Care Only (PCO) group visited primary care without chiropractic care or analgesic prescriptions.

Vertebrobasilar dolichoectasia (VBD) is a vascular anomaly marked by abnormal elongation and dilation of the vertebral and basilar arteries. Often, VBD remains undiagnosed or is discovered incidentally during evaluations of vascular events such as ischemia, hemorrhage, hydrocephalus, or cranial nerve palsies. While most patients are managed conservatively, treatment choices are highly individualized based on clinical presentation, vessel characteristics, and risk factors.

Thrombus enhancement sign (TES) is a potential imaging biomarker for differentiating embolic large vessel occlusion (embo-LVO) from intracranial atherosclerosis-related LVO (ICAS-LVO) in basilar artery occlusion (BAO). This study evaluates the association between TES and BAO etiology and its predictive value in distinguishing embo-LVO from ICAS-LVO. Patients with vertebrobasilar dolichoectasia (VBD) can present with cranial nerve deficits, such as cranial rhizopathies. Unlabelled: Neurovascular compression syndrome (NVCS) in the region of the upper thoracic aperture (UTA) occurs due to significant compression of the vascular-nervous bundle (VNB) in narrow anatomical spaces, which are represented by an interscalene space, costoclavicular space and area under the pectoralis minor tendon. Based on the data of traditional clinical and instrumental research methods, it is not always possible to accurately determine the nature and level of damage to the vascular-nervous bundle (VNB), that is often caused by variability of anatomical organization of the peripheral nervous system.
Objective: To identify and describe the specific clinical features of NVCS in the region of UTA and propose an informative instrumental method of diagnosis for development of the optimal surgical treatment algorithm.
