Article Abstract

Vascular clamps are often used during microvascular repair and anastomosis. However, pressure exerted by the clamps may damage the vessels, which compromises patency of vessels. This article reports on a microsurgical suture technique performed without any clamp to avoid clamp-related problems.

Background: Digital nerves provide sensibility to the fingers. They are commonly injured through accidental sharp laceration. The aim of the NEON (Nerve rEpair Or Not) study was to investigate whether microsurgical suture repair of lacerated digital nerves is superior to nerve alignment alone without suture repair.

Robotic surgery has introduced significant innovations in hand microsurgery, enabling complex procedures such as nerve sutures and arterial and venous anastomoses to be performed with greater precision and control. The Symani® Surgical System represents a promising advancement over conventional techniques by offering enhanced dexterity, tremor elimination, and motion scaling to improve microsurgical outcomes. While most studies in the literature focus on clinical comparisons between traditional and robotic approaches, our work aims to explore the practical aspects of integrating the Symani® into hand surgery.

The aim of the study was to determine the efficacy of wrapping nerve suture over recovery. In this prospective study, 72 patients with injured digital collateral nerves were studied during six months after microsurgical repair. 28 patients had nerve epiperineural suture with vein conduit wrapping and 44 patients had simple epiperineural nerve suture.

Mastering microsurgical skills is essential for neurosurgical trainees. Video-based analysis of target tissue changes and surgical instrument motion provides an objective, quantitative method for assessing microsurgical proficiency, potentially enhancing training and patient safety. This study evaluates the effectiveness of an artificial intelligence (AI)-based video analysis model in assessing microsurgical performance and examines the correlation between AI-derived parameters and specific surgical skill components.
