Wrist biomechanics remain incompletely understood due to the complexity of experimental measurements in this multi-bone joint system. Finite element analysis provides a powerful alternative for investigating internal variables such as carpal kinematics and displacement patterns. This technical brief compares two bone representation approaches, all-cortical versus cortical-trabecular, using two distinct finite element models developed from the same wrist CT dataset.

Introduction: Dynamic CT imaging is a promising modality for evaluating wrist pathologies like scapholunate ligament (SL) injuries. The primary objective of this study is to extract carpal angles from dynamic CT datasets using an automated motion analysis algorithm to provide reference values for healthy wrist motion. Secondly, the feasibility of this automatic method to detect SL ligament pathology was evaluated.

Scapholunate (SL) instability remains a challenging condition with significant possibility of implications for wrist function and long-term outcomes. This review explores the limitations and complications of current trans-osseous scapholunate ligament (SLL) reconstruction techniques, focusing on isometric reconstruction challenges and complications, for example osteonecrosis, tunnel fractures, graft failure, and iatrogenic extrinsic ligament injuries. Through biomechanical analysis and clinical case reviews, we demonstrate that the conventional three-ligament tenodesis (3LT) scaphoid tunnel has high risk of resulting in a non-isometric volar scapho-trapezial (vST) ligament reconstruction, potentially exacerbating carpal instability.

Purpose: Kienböck disease is characterized by lunate osteonecrosis with progressive carpal collapse. The relative benefit of revascularization compared to other surgical options remains uncertain. The purpose of this investigation was to compare long-term radiographic, functional, and patient-reported outcomes of revascularization techniques to alternative surgical procedures for the treatment of early- and late-stage Kienböck disease.
