Article Abstract

During epidural needle placement in a 32-kg dog the subarachnoid space was punctured and half the intended dose of lidocaine, bupivacaine, and morphine was injected. After recovery from anesthesia the dog showed signs of severe pruritus of the tail base and limbs and myoclonus of the tail and hind limbs. Methadone, acepromazine, ketamine, buprenorphine, and butorphanol were administered to control myoclonus and pruritus, but were unsuccessful. Diazepam was used to control myoclonus until the effects of morphine abated. Myoclonus Dystonia is a Mendelian inherited, childhood-onset dystonic disorder, caused by mutations in the autosomal dominantly inherited SGCE gene, and in which both motor and psychiatric phenotypes are observed. Results from murine and in vivo human studies suggest dystonia is caused by disruption to neuronal networks, and in particular the basal ganglia-cerebello-thalamo-cortical circuit. Work focused on the cortical component implicates disruption to neuronal excitatory/inhibitory balance as being a key contributor in the observed phenotypes.

Objective: This study was undertaken to determine the thalamic nuclei that are different between juvenile myoclonic epilepsy (JME) and healthy controls from the Juvenile Myoclonic Epilepsy Connectome Project and then to determine their relationship with other subcortical gray matter volumes, disease covariates, and motor performance.
