Article Abstract

The unicuspid aortic valve is an extremely rare congenital anomaly. It usually presents with aortic stenosis and/or aortic regurgitation. Other cardiovascular complications, such as aortic dilatation and left ventricular hypertrophy can accompany it. Herein, we present a case report of a 50-year-old asymptomatic male patient with unicuspid aortic valve, complicated by ascending aortic aneurysm.

Background: Initial treatment failure in young patients with aortic valve disease requires effective strategies beyond prosthetic replacement. This case demonstrates the Ross procedure (RP) as salvage for a failed valve reconstruction.

Case Summary: A 21-year-old man with a unicuspid aortic valve, severe regurgitation, and root aneurysm underwent valve-sparing root replacement with an acceptable intraoperative result.

Unicuspid aortic valve (UAV) is a rare congenital anomaly, diagnosed in individuals aged 30 to 50. It is associated with aortic stenosis or aortic regurgitation, potentially leading to heart failure. We report a case of a 28-year-old male with an acommissural UAV, aortopathy, critical aortic stenosis, and heart failure with reduced ejection fraction, presenting with shortness of breath.
