Article Abstract

A case of a sigmoid perforation presenting as a pneumoscrotum with progressive surgical emphysema is reported. An elderly patient presented with a grossly swollen scrotum and a distended abdomen. A computed tomography (CT) scan demonstrated sigmoid diverticulitis in conjunction with air and a subdiaphragmatic collection. A laparotomy was performed and revealed a perforated sigmoid diverticulum (Hinchey III). Sadly the patient died on the operating table. Intra-abdominal pathology manifesting as a pneumoscrotum and surgical emphysema is a rare yet reported phenomenon most often associated with colonoscopy, but also described with appendicitis and perforated colonic carcinoma. In previous reports, in contrast to this patient, the individual's scrotum was inflamed but not erythematous and tender, and the rapid progression of surgical emphysema is not reported.

Minimally invasive laparoscopic surgical techniques are desirable in horses and other equids. A new approach called "modified single incision laparoscopic surgery (SILS) for equine cryptorchidectomy" is described, along with the postoperative outcomes of 10 equids. A SILS Port device (Covidien) was placed a 30- to 40-millimeter incision in combination with 1 or 2 15-millimeter para-inguinal incisions to exteriorize the intra-abdominal testicles.

I report 2 cases of pleurodesis with 50% dextrose water (DW) in very elderly patients with secondary spontaneous pneumothorax. In both cases, a chest computed tomography scan showed a large pneumothorax with emphysema and multiple bullae. Patients were expected not to tolerate surgical treatment, considering their old age and underlying pulmonary disease.

Giant bullous emphysema (GBE) is a rare condition marked by irreversible damage to the lung parenchyma. Its clinical presentation ranges from asymptomatic to various respiratory symptoms.
