Objectives: Laparoscopic (lap) paraesophageal hernia repair has excellent short-term outcomes but higher long-term recurrence rates compared with the transthoracic repair. We hypothesized that the robotic-assisted lap (robot) approach would have similarly good short-term outcomes as lap, but also lower recurrence rates.
Methods: A retrospective study of prospectively collected data was performed for paraesophageal hernia repairs at a single high-volume quaternary hospital from July 2018 to September 2022.

Clinical Condition: A 35-year-old healthy male veteran presented with angina, shortness of breath with exertion, and near-fainting episodes. A transthoracic echocardiogram showed a septated cyst compressing the left ventricle. Cardiac magnetic resonance imaging and computed tomography angiography confirmed the presence of a pericardial cyst measuring 7.

Surgical approaches for thoracic esophageal cancer have evolved from invasive open procedures to minimally invasive techniques such as thoracoscopic and robot-assisted surgery. While robotic surgery offers improved precision and visualization, it still relies on single-lung ventilation and may not significantly reduce postoperative complications. Recently, transcervical esophagectomy (TCE) using a mediastinoscope has emerged as a promising alternative, enabling esophageal resection and lymphadenectomy via the neck without thoracic incisions.

Oesophageal lipoma is a benign oesophageal tumour accounting for less than 1% of all the gastrointestinal lipomas. The spindle cell variant of oesophageal lipoma is an even rarer entity. Histologically it is characterized by the presence of varying amounts of mature fat, uniform spindle cells and collagen.

Rationale: Coronary vasospasm (CAS) is often triggered by autonomic imbalance and can lead to severe cardiac events, particularly in patients with undiagnosed variant angina. Mannitol, commonly used for renal protection, may inadvertently activate the sympathetic nervous system, potentially leading to CAS in susceptible patients. This rare case highlights the potential cardiac risks of mannitol-induced CAS during robotic-assisted surgery.
