Article Abstract

The combination of periodic alternating nystagmus (PAN) and periodic alternating skew deviation (PASD) is rare. We report a case of PAN and PASD in a patient with spinocerebellar ataxia type 6 (SCA-6) and discuss the role of the cerebellum as a plausible mechanism for this combined pathologic condition.

Background: Immune checkpoint inhibitors (ICIs) are clinically beneficial but associated with high costs that represent a growing challenge for healthcare budgets and may affect affordability, especially in resource-limited settings. Moreover, the healthcare sector is a significant source of greenhouse gas emissions, and medication-related waste-such as that from vial-based therapies-has been identified as a contributing factor. Vitiligo is a chronic skin disease characterized by white patches caused by the destruction of melanocytes. The most well-known variant is non-segmental vitiligo, where patches are symmetrically distributed across the entire body, with alternating periods of stability and progression. The white patches arise due to an autoimmune reaction in which cytotoxic T-cells attack the melanocytes.
