In this paper, we investigate the applicability of experimental model ecosystems (microcosms and mesocosms) for the ecological risk assessment of veterinary medicinal products (VMPs). VMPs are used in large quantities, but the assessment of associated risks to the environment is limited, although they are continually infused into the environment via a number of routes. It is argued that the experience obtained by pesticide research largely can be used when evaluating VMPs, although there are several major differences between pesticides and pharmaceuticals (e.g., knowledge of their mechanisms of action on nontarget organisms). Also, because microorganisms are often the target organisms of VMPs, risk assessment should focus more on endpoints describing functional processes. This paper provides a review of the current risk assessment schemes of Europe and North America along with examples of experiments already performed with veterinary medicinal products in aquatic and terrestrial ecosystem models. We suggest that some of the approaches developed for pesticide risk assessment can be used for VMPs and offer suggestions for the development of a framework for ecological risk assessment of VMPs.

Study Question: Does weight loss from a hypocaloric dietary intervention improve antral follicle dynamics in women with PCOS?
Summary Answer: During a 3-month hypocaloric dietary intervention, women with PCOS who experienced clinically meaningful weight loss showed more organized antral follicle development including fewer recruitment events, but no change in the overall frequency of selection, dominance, or ovulation.
What Is Known Already: There is a spectrum of disordered antral follicle development in women with PCOS including excessive follicle recruitment and turnover, decreased frequency of selection and dominance, and failure of ovulation. Lifestyle intervention aimed at weight loss is recommended to improve metabolic health in women with PCOS yet benefits on ovarian follicle development and ovulation are unclear.

Background: Cirrhosis is a leading cause of noncancer deaths in gastrointestinal diseases, resulting in high hospitalization and readmission rates. Early identification of high-risk patients is vital for proactive interventions and improving health care outcomes. However, the quality and integrity of real-world electronic health records (EHRs) limit their utility in developing risk assessment tools.

Objective: To describe the sociodemographic and clinical characteristics of individuals exposed to smoking or biomass smoke and followed at primary health care (PHC) centers across three states in Brazil.
Methods: This was a cross-sectional multicenter study including patients followed at any of four PHC centers in Brazil. Patients ≥ 35 years of age who were smokers or former smokers, or were exposed to biomass smoke were included, the exception being those with physical/mental disabilities and those who were pregnant.

Objective: To develop a score (Palineo score) to identify the palliative care needs of newborn patients admitted to a Brazilian neonatal intensive care unit of a tertiary maternity hospital that serves as a reference center for high-risk pregnancies, ensuring timely follow-up by a specialist.
Methods: Patients were assessed by three specialists using a questionnaire that included the same clinical elements as those used for the Palineo score but did not assign scores to the criteria. The score was determined by the consensus reached by the specialists.
