Introduction: Thyrotoxicosis is a hypermetabolic state that can be managed with antithyroid drugs, radioactive iodine (RAI), or thyroidectomy. However, the long-term cardiovascular effects of these treatments remain unclear. This study evaluates surgical versus nonsurgical approaches on new-onset cardiovascular complications in thyrotoxicosis patients.

Introduction: Thyroid nodules are common and require vigilant monitoring due to malignancy potential. They become particularly concerning in patients using glucagon-like peptide (GLP-1) analogs, which are associated with a proposed increased risk of medullary thyroid cancer. This study aims to evaluate the incidence of thyroid cancer in patients with thyroid nodules treated with GLP-1 analogues.

Introduction: Previous studies from single-institution cohorts have shown that patients with normocalcemic primary hyperparathyroidism (ncPHPT) are just as symptomatic as patients with classic PHPT. There is currently no large-scale database analysis of ncPHPT, so with the TriNetX database, a large international database, trends on ncPHPT can be studied over a large cohort of patients. The aim of this project is to characterize the diagnostic and treatment paradigm for ncPHPT in the TriNetX database.

Background: Radioguided parathyroidectomy is based on the principle that hyperfunctioning parathyroid glands have increased radiotracer uptake, which can be measured instantaneously. We sought to determine if ex vivo radioactivity measurement could be used to predict parathyroid pathology and guide surgical decision-making in a time-efficient manner. Materials and Methods: We retrospectively reviewed patients with primary hyperparathyroidism who underwent parathyroidectomy at our institution (2000-2022).

Background: Although most patients with intrathoracic thyroid glands can undergo thyroidectomy via a cervical incision, some cases necessitate intraoperative thoracic surgery assistance. This study aimed to identify CT findings that predict the need for thoracic surgery assistance during thyroidectomy.

Study Design: We reviewed administrative data at our institution (2012 to 2023), encompassing 7,370 thyroidectomies, to identify patients who underwent thyroidectomy for intrathoracic thyroid glands, defined as thyroid glands that extended below the sternal notch.

Objective: To assess the impact of parathyroid gland autotransplantation on the restoration of parathyroid function in patients who are hypoparathyroid after thyroidectomy.

Background Data: Hypoparathyroidism post-thyroidectomy arises when all parathyroid glands are devascularized or injured. Autotransplantation of compromised parathyroids aims to preserve their function and prevent permanent hypoparathyroidism.

Introduction: Hypocalcemia occurs in 20%-40% of total thyroidectomy cases, traditionally requires 1-2 ds of hospitalization for management. This study examines the extent of hypocalcemia following a postanesthesia care unit (PACU) parathyroid hormone (PTH)-based protocol after outpatient thyroidectomy.

Methods: Patients who underwent total or completion thyroidectomy for non-Graves' disease at a single institution between December 2015 and September 2023 were included.

Article Synopsis
  • Hypercalcemia can be caused by hyperparathyroidism or non-parathyroid conditions, with undetectable parathyroid hormone (PTH) levels making hyperparathyroidism less likely.
  • A 65-year-old woman presented with hypercalcemia, fatigue, and brain fog; extensive testing ruled out common causes, and an ultrasound suggested an enlarged parathyroid gland.
  • Treatment with cinacalcet reduced her calcium levels, and after surgery to remove a 1927 mg adenoma, her levels normalized and symptoms improved, indicating that the adenoma may have a genetic mutation leading to low PTH production.
