Objective: Indications for TAVI have been widened, although long-term device efficacy is still unclear. We aimed to compare the effectiveness of transcatheter aortic valve implantation (TAVI) vs. surgical aortic valve replacement (SAVR) on a composite of death from any cause or stroke at 5-year follow-up according to risk profiles.

Background: Coronary artery bypass grafting (CABG) remains a cornerstone treatment for multivessel coronary artery disease. While single-artery revascularization has been the standard, the use of multiarterial grafting (MAG) is associated with superior long-term outcomes. However, its adoption is limited by technical challenges and potential complications.

Objectives: The commonly accepted aortic valve prostheses have been either mechanical or biological. Each type has its advantages and disadvantages, with age being the most widely accepted variable to determine the best option. There is, however, a range between 60 and 70 years where an individualized approach is required.

Objectives: There is general consensus of the higher short-term risk in women after coronary artery bypass grafts (CABG), nonetheless, long-term survival is a matter of debate. We aimed to compare in a national database with over 10 years of follow-up long-term survival in women versus men and its interaction with diabetes and age.

Methods: This is a national retrospective cohort study from Uruguay.

The structural and functional integrity of conduits used for coronary artery bypass grafting is critical for graft patency. Disruption of endothelial integrity and endothelial dysfunction are incurred during conduit harvesting subsequent to mechanical or thermal injury and during conduit storage prior to grafting, leading to acute thrombosis and early graft failure. Late graft failure, in particular that of vein grafts, is precipitated by progressive atherogenesis.

Objective: The best aortic prosthesis type in 60-70 year old patients is not established. Our aim was to evaluate the survival in a National cohort of patients between 60-70 years old who required surgical aortic valve replacement for aortic stenosis (SAVR) with either a mechanical (MP) or bioprosthesis (BP) valve.

Methods: This is a retrospective study using national data from the Ministry of Health.

Article Synopsis
  • - The integrity of conduits for coronary artery bypass grafting is crucial for their long-term success, as damage during harvesting and storage can lead to graft failure or thrombosis.
  • - Effective intra-operative management involves choosing the right techniques for harvesting and protecting the conduits, as well as using vasodilators to prevent vasospasm.
  • - Post-operative care includes continuing vasodilator therapy and using medications to prevent atherosclerosis progression, with the review highlighting both established practices and areas needing further study.

Article Synopsis
  • Cardiovascular diseases are a big problem around the world, and clinical trials help find better treatments for them.
  • Conflicts of interest (COIs), which can happen when companies have a say in research, can affect how trials are carried out and the results they show.
  • It's important to be careful about COIs in studies because they can change guidelines that doctors follow, and sharing rules and being open about research can help keep everyone’s trust in the science.

Article Synopsis
  • The DANAMI-3 DEFER study shows that postponing stent implantation in ST-elevation myocardial infarction (STEMI) is safe and may work well when carefully choosing patients.
  • The study included 198 patients undergoing primary percutaneous coronary intervention (PCI) and compared those who had deferred stenting with those who received immediate stenting.
  • Results indicated that while the deferred stenting group had lower rates of stent use and employed more thrombus aspiration, there were no significant differences in mortality or major adverse events between the two groups, suggesting the need for further research on tailored treatment strategies.

Background: Prevalence of bioprosthetic valve degeneration (BVD) is rising as the use of bioprosthetic aortic valves increases. Detecting early signs of BVD remains a challenge, with conventional imaging methods often failing to identify early deterioration stages. 18F-fuoride positron emission tomography (PET-CT) is an emerging technique that offers promising prospects to detect subclinical BVD.

Background: Most evidence for anticoagulation in aortic bioprosthesis is centered on embolic events, bleeding and re-intervention risk. The effect of anticoagulation on hemodynamics has not been previously assessed. Our hypothesis was that patients with anticoagulation (AC) early after aortic valve replacement (AVR) with porcine bioprosthesis have better hemodynamics at 3 years of follow-up.

Composite end points are common primary outcomes in clinical trials. Their main benefit of utilizing a composite outcome is increasing the number of primary outcome events, meaning fewer participants are required to deliver an adequately powered trial. By combining multiple important end points in the primary outcome rather than having to select only 1, composite end points potentially make clinically meaningful benefits easier to detect and avoid ranking outcomes hierarchically.

Objectives: The ISCHEMIA trial is a landmark study that has been the subject of heated debate within the cardiovascular community. In this analysis of the ISCHEMIA trial, we aim to set the record straight on the benefits of coronary artery bypass grafting (CABG) and the misinterpretation of this landmark trial. We sought to clarify and reorient this misinterpretation.
