Background: Lung transplantation remains the optimal treatment for children with end-stage lung disease, yet donor organ shortage represents the greatest obstacle to transplantation. In 2023, only 31 pediatric lung transplants were performed in the United States, with 9% of recovered lungs ultimately not transplanted. Pediatric waitlist mortality has increased, particularly for patients under one year of age, necessitating innovative strategies to expand the donor pool.

The rising popularity of electronic cigarettes (e-cigarettes), the nicotine product that is most used by adolescents since 2014, has reversed decades of progress in declining youth tobacco use. E-cigarette use in adolescents is associated with future smoking, and evidence is mounting of an increased association with nicotine dependence. Therapies used to treat nicotine dependence in adults include pharmacotherapy and behavioral interventions.

Case: A 7-year-old girl presented with a painful effusion in her right knee, fever, and a negative joint aspirate. One week later, she presented again with an effusion in her left elbow, elevated inflammatory markers, and a joint aspirate that raised concerns for septic arthritis. Irrigation and debridement of the elbow were performed, and the cultures returned negative results.
