Background: Todani Type IVa choledochal cysts are rare congenital malformations involving both intrahepatic and extrahepatic bile ducts. Adult-onset, asymptomatic, and localized presentations are exceptionally uncommon and pose a surgical challenge, particularly when considering the extent of resection and reconstruction required.

Case Presentation: We report the case of a 24-year-old asymptomatic female who was incidentally diagnosed with a localized Todani Type IVa cyst involving the left hepatic lobe and the common bile duct.

Background: Bezoars are gastrointestinal foreign bodies, often found in the stomach, composed of indigestible materials. Trichobezoars are rare gastrointestinal masses composed of ingested hair, primarily affecting young females with underlying psychiatric conditions such as trichotillomania and trichophagia. If left untreated, they can result in significant complications including: abdominal pain, gastric ulcers, gastrointestinal bleeding, obstruction, and even perforation.

Spindle cell lipomas (SCLs) are rare benign adipocytic tumors, predominantly occurring in elderly men's posterior neck, upper back, and shoulders. Accounting for 1.5% of lipomas, SCLs in atypical locations like the inguinal region are exceptionally rare and pose diagnostic challenges due to similarities with malignant tumors.
