Unlabelled: Acute liver failure due to widespread carcinomatous infiltration of the liver by breast cancer carries a grave prognosis. In this position statement the British and Irish Hypertension Society (BIHS) present a review of the current evidence for blood pressure (BP) treatment thresholds and targets. The BIHS recommend initiating pharmacological antihypertensive therapy, irrespective of cardiovascular disease risk, following a confirmed diagnosis of hypertension (sustained out-of-office BP ≥ 135/85 mmHg despite diet and lifestyle advice). The BIHS recommend an on-treatment BP target < 130/80 mmHg or as low as reasonably achievable without causing unacceptable side-effects, within 6-months of initiating treatment, for all adults.

Salivary duct carcinoma (SDC) is one of the rarest malignancies. It can arise de novo or from pre-existing pleomorphic adenoma (carcinoma ex pleomorphic adenoma - CXPA) and can be very aggressive. Advance care planning (ACP) involves longitudinal communication between the patient and physician to explore the patient's wishes and goals while relaying accurate diagnostic and prognostic information to support informed and shared medical decision-making. In pediatrics, it also uniquely involves the parents or legal guardians as the primary medical decision-makers. ACP ideally leads to the implementation of advanced directives (ADs) and can be a difficult concept to discuss with pediatric patients and families with refractory oncologic disease, given the distinctive burdens that accompany this population.

Limited randomized or prospective data are available to guide local/metastasis directed therapy (LMDT) in relapsed/recurrent Ewing sarcoma (RR-ES), resulting in uncertainty regarding best clinical practice for these patients. This report reviews the available literature on LMDT approaches and provides consensus recommendations regarding therapeutic decision making, timing, and indications for the use of LMDT in the management of RR-ES. LMDT should be considered on a case-by-case basis to assess appropriateness, optimal timing/modality, palliative versus curative intent, and its role in relation to chemotherapy.
