Background: Mediastinal hematoma is a rare complication of transradial access during invasive coronary angiography.

Case Summary: We present a case of a massive cervical-mediastinal hematoma after a complex percutaneous intervention that resulted in airway compromise and prolonged mechanical ventilation.

Discussion: We focus on multidisciplinary management and, given limited data in the literature, provide a pathway for a safe and efficient approach to management.

Antimicrobial prescribing differences between physicians and nurse practitioners (NPs) remain poorly characterized. We compared prescribing practices at a safety-net hospital. NPs adhered more to pneumonia guidelines, while physicians had better adherence for abdominal and urinary infections.

Whole-genome sequencing (WGS) is a highly sensitive method for identifying genetic relatedness and transmission of strains. Previous studies suggest that as few as 3 core genome single-nucleotide variants (SNVs) discriminate between genetically distinct isolates. Because a single colony is selected from culture for WGS, significant within-host genetic diversity could preclude identification of transmission events.
